Navigating Fertility with PCOS

PCOS is a common hormonal condition that can make getting pregnant tricky, but it’s totally possible with the right plan! It’s the leading cause of ovulation-related infertility and can impact hormone levels, metabolism, and menstrual cycles, complicating conception. 

Why Ovulation is Hard with PCOS 

Ovulation can be unpredictable in PCOS because of hormone imbalances, especially high LH  levels compared to FSH. This disrupts egg development and causes irregular periods. Standard ovulation kits may not work because they detect LH, which is naturally elevated in PCOS. Instead, try tracking basal body temperature (BBT) and/or cervical mucus. 

First Steps to Boost Fertility 

The first step to optimizing fertility chances in a woman with PCOS includes extensive history taking and bloodwork with cycle monitoring. Here is a list of lab tests that should be considered: 

  • Check for insulin resistance: fasting glucose, fasting insulin, HbA1C 

  • Check for cholesterol levels: lipid panel 

  • Check luteal phase hormones: progesterone and estrogen  

  • Check hormones: LH, FSH, AMH, Androgens (total testosterone and free testosterone) - Vitamin D  

  • Homocysteine

Treatment Options 

  • Conventional Treatments: Medications like letrozole, metformin, or clomiphene citrate can help trigger ovulation. If these don’t work, IVF may be an option.

  • Nutrition: Focus on balanced blood sugars and weight management. Even a 2–5%  weight loss can improve ovulation. However it is important to note that weight loss does not guarantee pregnancy. Treatments should not be delayed to meet certain weight  goals.
